ABSTRACT: In patients of all ages, many disorders labeled as psychiatric may actually
be due to hormonal insufficiencies. For example, cortisol deficiency is rarely
taken into account in a medical or psychiatric work-up, so persons with mild
to moderate cortisol insufficiency are for the most part relegated to receiving a
psychiatric diagnosis when, in fact, the same disorder is represented. However,
the symptoms of cortisol insufficiency appear to closely parallel such psychiatric
disorders as post traumatic stress disorder (PTSD) and addictions. There has
been some question of whether substance abuse causes a hypocortisolemic state.
In reviewing the literature and obtaining detailed histories of addicted patients,
it appears that childhood trauma, also known as “early life stress” (ELS), instead
may elicit a hypocortisolemic state. This leads some to self-medicate with an addictive
substance to quell the pain of a cortisol insufficiency, both physical and
emotional. In fact, the literature supports the concept that addictive substances
increase cortisol in predisposed patients. Patients with a variety of psychiatric
disorders including addictions were found to have signs and symptoms of mild or
moderate hypocortisolemia. Generally, an appropriate comprehensive examination
supported a diagnosis of cortisol insuffiency. For the most part, these patients
were succesfully treated with physiologic doses of bio-equivalent
hydrocortisone, along with replacement of any other deficient hormone. By correcting
underlying hormonal insufficiencies, many patients improved, with some
patients having a total reversal of psychiatric symptoms. It is therefore reasonable
to evaluate and treat hormonal insufficiencies with hormones prior to using
psychotropic medication.

Is Intervention Necessary?

If family members or loved ones wait until the individual is ready to quit drinking or taking drugs, they’ll likely be waiting until it’s too late. A professional intervention can be extremely helpful in breaking through the defenses of denial. Interventions typically involve friends, family members and a professional interventionist in a non-judgmental form of confrontation that invites the addict or alcoholic to seek treatment. Typically, interventions stand a better chance of being successful if conducted by people the individual knows and trusts. The person being confronted may feel cornered and get angry or defensive. An interventionist serves as a mediator, guiding communication between loved ones and the individual concerned.
Preparing for the Intervention

Drug Alcohol Detox Support

Detox Doesn't Mean You Have to Suffer

Initial withdrawal from drugs or alcohol can be physically distressing, painful and even dangerous. Without appropriate care, withdrawal symptoms can include increased blood pressure, irregular heartbeat, severe muscle cramping and seizures.

Alcohol Detox & Drug Detox

Certain types of drugs require a detox. Alcohol detox generally includes monitoring vital signs, doctor consultation and prescribed medication to ease withdrawal symptoms. Examples of drugs that require detox include: Alcohol, benzodiazepines (i.e., Valium, Xanax) and opiate-based drugs including heroin and prescription painkillers such as Oxycontin, Vicodin, Hydrocodone, etc.

Other drugs such as cocaine, methamphetamines and marijuana do not require detox. However, as with any drug dependent upon amount and frequency of use there remains the significant potential for psychological dependence and given this, a period of stabilization followed up by a recovery program is advisable for long-term success.

Alcohol detox

Alcohol poses one of the most dangerous forms of withdrawal. An abrupt cessation of alcohol (going “cold turkey”) will result in withdrawal symptoms within hours and the time necessary for complete alcohol detox can range from 3 to 14 days. Alcohol detox can result in a variety of physical and/or psychological symptoms. Mild withdrawal symptoms include headaches, nausea, vomiting, perspiration, restlessness, agitation, loss of appetite and insomnia. More severe symptoms may disrupt the autonomic system, resulting in seizures (convulsions), visual or auditory hallucinations and the DTs (Delirium Tremens). It is estimated that 1 in 4 patients are at a high risk for a withdrawal seizure if medications are not administered during the detox phase. For this reason, alcohol detox frequently involves doctor prescribed medication that have similar effects to alcohol are given to reduce the risk of seizure. These medications generally include certain benzodiazepines and anticonvulsant medication that are prescribed in tapering amounts until the body is physically stabilized and the threat of seizure removed. Professional advice should always be sought in alcohol detox and consultation with a doctor during alcohol detox is generally necessary.

Specific Types of Drug Detox

Benzodiazepine Detox

Benzodiazepines (Valium, Xanax, Klonapin) are classified as depressants, tranquilizers or sedatives as they slow down central nervous system activity, Benzodiazepines can become addictive within a short period of time and are one of the most dangerous drugs to detox from due to the high risk of seizure.
Opiate Detox

Opiates such as heroin, methadone and prescription painkillers generally require a detox depending upon quantity and frequency of usage. Dramatically reducing or stopping opiate-based drugs after prolonged and/or heavy use frequently results in a very difficult withdrawal. While there are many types of detox options available, supervised detox is highly recommended for safe withdrawal.
Meth detox

Methamphetamines can either be snorted, smoked, or injected bringing a powerful adrenaline rush, excitement, exhilaration and euphoria. Meth has been described as an intense and nearly instant addiction. Detoxification from methamphetamines does not require supervision; however, the cravings can be intense. For this reason, a cutting-edge technique known as Neruo-Transmitter Restoration Therapy (NTR) has been introduced as an effective way to reduce and potentially eliminate the often intense cravings. NTR is a naturally-occurring amino acid solution delivered intravenously that bathes the brain in nutrients, easing withdrawal symptoms and cravings and restoring a natural alertness within days instead of the weeks or months it can take under normal detoxification procedures.
